Diaconal Ministries Resource Manual 17 Samaritan House - Ft. Collins T he Samaritan House – Ft. Collins, a facility of Catholic Charities Northern, provides nightly shelter, breakfast and dinner to homeless single individuals and families. They provide meals for those in need through the hospitality kitchen. Other services include: emergency assistance for utilities, bus passes, along with a Senior Program, and a Veterans' Program. Deacons lead liturgy and prayer services during the week for residents and staff members. They interact with the residents during their visits and assist with meal preparation including family night once a month. i Deacon Dennis Wallisch denwallisch@comcast.net 970.443.5695 Deacon Greg Reynolds gcreynolds@stjosephfc.org 970.388.9949 Guadalupe Community Center T he Guadalupe Community Center is a homeless shelter in Greeley which offers programs and services that operate on a "support to succeed" approach where homelessness does not equate hopelessness. The Center gives these residents a safe and supportive place to live for a few months while they secure income and save the money to transition to affordable housing. Many people who have lost their jobs and their homes also become lost in their faith. The deacon's presence and support provides a means for them to rediscover their faith life. A chapel is located within the Center and provides a space for clergy to meet residents for consultations and prayer. Ministry to the residents of the Guadalupe Community Center in Greeley consists of deacons assisting with the spiritual needs of the residents by providing spiritual counseling, providing a Catholic presence, and a Communion service every other week. A Mass is also celebrated once a month for residents and staff. i Guadalupe Community Center 970.353.6433 Boulder Shelter for the Homeless B oulder Shelter for the Homeless pro- vides services for residents by focusing on finding stable housing with case man- agement support and referrals with outside agencies. The Shelter has four programs. Coordinated Entry, where residents meet with a staff person and complete a brief assessment before being referred to a local service provider for comprehensive sup- port. Housing Focused Shelter includes case management, counseling, medical care, and Veterans Affairs (VA) resources are provided along with short-term housing. Permanent Supportive Housing provides intense case management to overcome chronic home- lessness. Rapid Re-housing assists in finding housing in the community. The deacon is the Program Manager for the Permanent Supportive Housing Pro- gram.
